As of March 1, 2010, University Health Plans, Inc. (UHP) is no longer providing services to NJ FamilyCare/Medicaid program. In February, you should have received a new HMO Identification (ID) card from your new HMO. The new ID card became effective March 1, 2010. If you have received an Amerigroup ID card please contact Amerigroup at (800) 600-4441. If you selected another HMO please contact that HMO for assistance. If you are uncertain as to what HMO you are enrolled with please contact NJ Family Care for assistance at (800) 701-0710.

Voluntary Members

Eligible members may select Medicaid Fee-for-Service if you do not want to be a member of another health plan. If you choose to make this move, please contact the State’s Health Benefits Coordinator at (866) 472-5338 (TTY: (800) 701-0720).
